How Maintenance Fluid Calculation Works
Our maintenance fluid calculator uses the Holliday-Segar method (4-2-1 rule), the gold standard for calculating baseline IV fluid requirements. This method accounts for normal metabolic water losses through urine, stool, and insensible losses.
The 4-2-1 Rule Formula
(10 kg × 4) + (10 kg × 2) + (5 kg × 1) = 40 + 20 + 5 = 65 mL/hr
This stepwise calculation reflects decreasing metabolic rate per kilogram as body weight increases. The method was developed by Holliday and Segar in 1957 and remains the standard for maintenance fluid calculation.
Clinical Application Guidelines
Maintenance fluids replace normal daily losses in patients who cannot maintain oral hydration:
- Indications: NPO status, surgery, severe illness, vomiting, inability to drink
- Fluid Type: Isotonic fluids preferred (D5 0.9% NaCl or D5 0.45% NaCl with KCl)
- Potassium: Add 20 mEq/L KCl after adequate urine output confirmed
- Monitoring: Urine output 0.5-2 mL/kg/hr, daily weights, electrolytes q12-24h
- Adjustments: Increase for fever (+10% per °C), ongoing losses, increased metabolism
- Contraindications: Not for fluid resuscitation, shock, or replacing large deficits
- Reassessment: Adjust rates every 8-12 hours based on clinical status
